The Health Centre will be closed for staff training from 1 p.m. to 6 p.m. on Wednesday 28th February and Wednesday 28th March.  In the event of a medical emergency which cannot wait until we re-open, please ring 01382 424076 and you will be connected to the Tayside Primary Care Emergency Service.

New Patient Registration


There are two medical practices at the Health Centre:-

  • Orwell Medical Practice
  • St Serf's Medical Practice

Patients may choose which practice they wish to join.  Information on each practice is available on-line, see Doctors, or from Reception.  Patients register with a practice, but may express a preference to see a particular GP.  This preference may be expressed on booking appointments.

All new patients must complete the following forms:-

  • Registration Form
  • Medical Questionnaire

These may be completed on-line or downloaded.  See Registration Forms.

New patients will also be invited to attend a New Patient Medical with one of the Health Care Assistants.   The Health Care Assistants will ask for information about the patient’s present state of health, any medication being taken, allergies, lifestyle and relevant family medical history.  It is helpful to the clinicians if this appointment is attended prior to seeing any of the GPs or practice nurses.

Medical treatment is available from the date of registration.

Two forms of proof of identity must be produced to register.  One of these must include a photograph.  Acceptable forms of identification include:-

  • Passport
  • Drivers license
  • Birth Certificate
  • Utility bill
