There are two medical Practices at Loch Leven Health Centre:
- Orwell Medical Practice
- St Serf’s Medical Practice
You may only register permanently with a GP practice if you are are staying within the practice boundary for more than 3 months.
All new patients must complete new patient registration forms (GPR form and Practice New Patient Questionnaire for the practice of your choice). These can be obtained via the links below.
Acceptance is at the discretion of the Practice and we will ask you for proof of identity. (Please refer to our registration policy below). Medical treatment is available from the date of registration.
Your medical records may take some time to arrive so please ensure you have supplies of your repeat prescription items to last at least one month.

Step 1: Complete online registration form
The online registration form will collect your details, a brief outline of your medical history and record your preferences.
Step 2: Upload scans of photo identity and proof of address
Please provide two forms of your identity.
One must contain your photograph and the other must provide evidence of your current address.
