Private non-NHS Services (outwith the NHS contract)

We aim to support our patients but we will always prioritise NHS clinical work. We therefore cannot guarantee a timescale for completion of non-NHS services such as medical reports, letters, etc.  If you require this type of service urgently or if it is required within a specified timescale we would recommend that you seek to have this undertaken by an alternative person or organisation.

The NHS provides most healthcare to most people free of charge, but there are exceptions. GPs are self-employed and are contracted and paid by NHS Scotland to provide NHS general medical services for their patients.

GPs are frequently asked to provide additional services that fall outwith their contract with the NHS; in these circumstances, they are entitled to charge a fee. Fees are based on the British Medical Association (BMA) suggested scales and are applicable from 1 August 2023. https://www.bma.org.uk/pay-and-contracts/fees/why-doctors-charge-fees/why-does-my-doctor-charge-fees

Payment is required before private work is carried out.

Your questions answered

Isn’t the NHS free?

The National Health Service provides most healthcare to most people free of charge, but there are exceptions: for example, medical reports for insurance companies.

Surely the Doctor is being paid anyway?

In most GP Practices in the UK the NHS does not employ GPs. They are self-employed and must cover their business operating costs – staff, buildings, heating, lighting, etc, in the same way as any small business. The NHS does not pay these overheads.

Increasingly, organisations are involving Doctors in a wide range of work that is not directly related to the health of their patients. Often, the only reason that GPs are asked to do non-medical work is that they are in a position of trust in the community, or because an insurance company or employer wants to be sure that the information provided is true and accurate.

Why can it take my GP a long time to complete my form?

Time spent completing forms and preparing reports takes GPs away from direct medical care of their patients and they will always prioritise this over any private work. Provision of direct medical care is highly unpredictable and this often causes significant changes to a GP’s work plan.

I only need the Doctor’s signature……….

When a Doctor signs anything, it is a condition of remaining on the Medical Register that they only sign what they know to be true. To complete even the simplest form, the Doctor may have to check the patient’s entire medical record which can contain thousands of medical entries as it spans the whole of a patient’s life. That all takes time and is done in their own time, outwith their NHS contract.
