Completion of forms, certificates and medicals are not covered under the NHS contract and will often attract a fee. Payment must accompany the request or appointment booking.
Charges are aligned with the BMA’s guidelines. Please read the form carefully and fill in and sign any parts that you need to complete before you bring it to the Practice.
